Poland is a country located in the center of Europe on the shores of the Baltic Sea, where a huge number of architectural and historical monuments are concentrated. However, the demographic indicators of Poland have been disappointing lately. Poles are increasingly leaving to work and study in other countries, where, in their opinion, they have more prospects and opportunities. The government of the country has decided to rectify the current situation at the expense of ethnic Poles: the Polish Republic has adopted and operates a law on the Pole's Card, which deals with the provision of assistance to Poles living outside the country in order to preserve and strengthen the national tradition. In order to receive this document, you need a form for the Pole's Card, which must be filled out correctly.

What is a Pole's Card and how to get it

Karta polaka is a document confirming that a person belongs to the people of the Polish Republic. It should be borne in mind that, according to Polish law, the concepts of “Polish people” and “Polish citizenship” are not identical.

The cardholder receives the following rights and privileges:

  • the right to apply for a multiple entry visa to Poland without providing additional documents and paying fees;
  • not issue an invitation to work and a work visa for employment;
  • to conduct business on equal terms with the Poles;
  • receive free education and scholarships in Poland;
  • permanent discount of 37% on rail travel within the country;
  • free admission to state museums in Poland;
  • in urgent cases, the right to receive free medical care;
  • the right to apply for assistance to financial institutions located in other countries.

To obtain a Pole's Card, you need to prepare a package of necessary documents and apply to the Polish consulate.

Moreover, a Pole's Card can even be issued to a child: if the mother and father or at least one of the parents have this document (in this case, the consent of the other parent, given in the presence of the consul, is required), the child has the right to receive the card.

If one of the parents is deprived of parental rights, then his permission is not required. Persons who have reached the age of 16 must consent to the issuance and receipt of the Pole's Card.

General rules for filling out the questionnaire

An application form for a Pole's Card is a generalized document that contains detailed information about the applicant, his Polish relatives, as well as the grounds for issuing the document.

Basic rules for filling out the questionnaire:

  • information in the questionnaire is entered in Polish, with the exception of points 1 and 2;
  • one cell is one letter, number or symbol;
  • corrections are prohibited;
  • a check mark is used as a mark.

An application for a Pole's Card is drawn up in any form addressed to the consul of the Republic of Poland.

At the same time, the following data must be indicated in it: the surname, name and patronymic of the applicant, date and place of birth, gender, place of residence, citizenship, nationality, as well as the citizenship and nationality of relatives - parents, grandparents, great-grandmothers and great-grandfathers.

In addition to the application to the consulate at the place of residence of the applicant, it is also required to provide a document proving his identity (passport), as well as copies of papers that confirm the information specified in the application. These can be Polish identity papers, civil status certificates or their photocopies, birth certificates, school certificates, military service documents, etc.

Step-by-step instructions for filling out an entry on the Pole's Card

The questionnaire (or wniosek) will definitely be required at the interview with the consul.

1-2 You need to enter your last name and first name, as it is written in the civil passport, but in large block letters.

3-4 Enter your last name and first name in Polish.

5-6 Enter the full date of birth (day, month, year). In line number 6, indicate the gender.

  1. Indicate your place of birth in Polish. Moreover, it is not necessary to indicate whether it is a city, an urban-type settlement or a village.

8-9 Indicate your citizenship. On the next line, enter your nationality. Those wishing to receive a card by nationality must be Poles.

  1. Those who live in cities do not indicate anything in this line. Those who do not live in the city write obwod.
  2. Enter the registration index.
  3. Those who are registered in the city do not need to enter anything. The rest enter the name of the area.
  4. Indicate the name of the city or village without specifying the district, region, etc.

14-15 Indicate in Polish which street you live on. In the next line, enter the number of the house, building (if any) and apartment. All numbers are written with a slash (/).

  1. Enter the name of the country of residence.
  2. Here you need to indicate the name of the document (passport), which confirms your identity.
  3. Indicate the series and number of the passport.
  4. Enter the date of issue of the passport in the field.
  5. Enter the date until which the document is valid.
  6. In the first line, indicate the name of a Pole relative, in the second line - his surname.
  7. Place a check in the box next to the name of the relative you are related to.
  8. The citizen's citizenship is indicated at the moment or entered in the documents (in case of his death).
  9. Enter nationality: Polish.

25-26 Do not enter anything if the Pole Card is issued for an adult.

27-30 Enter data on the second relative, if you follow the line of a great-grandmother or great-grandfather. The data is entered in the same way as in paragraphs 21-24.

31-32 Do not enter any information if the Pole Card is issued for an adult.

  2. Check the Tak box if you received secondary or higher education in Poland.
